29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about 5/20/2016, at the above addressed jobsite, an employee was engaged in repairing a roof at approximately 9'-10" feet above ground level without a means of fall protection.    NOTE: CERTIFICATION OF ABATEMENT MUST BE PROVIDED FOR THIS ITEM.    MGC Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1020833, citation number 1, item number 1 and was affirmed as a final order on 8/26/2015, with respect to a workplace located at 6004 Royal Poinciana Ln, Tamarac, FL 33319.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support:       On or about 5/20/2016, at the above addressed jobsite, a ladder was not secured to a rigid support while extending 2 feet above a roof, exposing an employee to a fall hazard of 11'-7".
