29 CFR 1926.303(b)(2): The safety guard was not mounted so as to maintain proper alignment with the wheel, and/or the strength of the fastenings did not exceed the strength of the guard: On or about 5/23/2016 at 7510 Industrial Highway, Macon, GA: It was determined that the employee(s) operating or working near the area on 5/16/2016, were exposed to a struck-by hazard from the unguarded abrasive wheel used on the portable electrical hand grinders. On 5/16/2016 an employee was injured from the unguarded abrasive wheel exploding.
