29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  a) Frontier Hot Dip Galvanizing - On or about 5/20/16, employees operate various fork trucks every day to move racks of parts and large parts.  Employees have not received the required training nor were the operators' performance evaluated in the workplace.  ABATEMENT CERTIFICATION REQUIRED

Any employee was allowed to operate the cranes after general information from more senior personnel.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Frontier Hot Dip Galvanizing - Employees voluntarily wear Willson filtering face-piece particulate respirators for protection against zinc dust and other dusts every day.  The employees were not provided with the information contained in Appendix D of the respiratory protection standard.  ABATMENT CERTIFICATION REQUIRED
