29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section:      A.) Humphrey Charcoal Corporation - On or about 5/25/16, and at times prior thereto, employees were utilizing a moveable set of stairs to access the loading dock.  The stairs did not have a standard stair railing on the left side and the right side railing was damaged.  In addition, the railing did not extend to the top of the dock to provide an adequate handhold for the employees.      B.) Humphrey Charcoal Corporation - On or about 5/25/16, and at times prior thereto, employees were utilizing a permanent set of stairs to access the loading dock.  The stairs did not have a standard stair railing on the right side.

Humphrey Charcoal Corporation; briquette mill - On or about 5/25/16, and at times prior thereto,  moving parts  on the briquette mill were not guarded.  The unguarded moving parts including but were not limited to the briquette pan.    B.)  Humphrey Charcoal Corporation; briquette mill - On or about 5/25/16, and at times prior thereto,  the end of the conveyor of the briquette mill was not guarded.  The condition exposed employees to the nip point created by the belt conveyor.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:      A.)  Humphrey Charcoal Corporation; briquette mill - On or about 5/25/16, and times prior thereto,  employees were reaching into an area near the briquette mill pan to scoop out charcoal mixture onto the conveyor.  The employees were not using any types of special tooling to keep their hands out of the moving parts.   Moving parts included, but were not limited to, the briquette mill pan.

Humphrey Charcoal Corporation; outside break room - On or about 5/25/16, and at times prior thereto, outside wiring that powers various things including, but not limited to the mill, was  located less than 10 feet of the ground.  The wire connections outside of the break room/ maintenance area were within 7 feet of the ground.

Humphrey Charcoal Corporation; weight bagger platform - On or about 5/25/16, and at times prior thereto, employees were exposed to an unguarded belt and pulley system that was 4 feet above the level the work platform.        Humphrey Charcoal Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.219(d)(1)), which was contained in OSHA inspection number 916884, citation number 1, item number 5 and was affirmed as a final order on 1/13/14, with respect to a workplace located at 13760 Know Dale Road, Brookville, PA.
