Humphrey Charcoal Corporation, on or about 2/28/25, and times prior thereto: Employees were working around exposed rotating parts located approximately 1 foot above the floor. The rotating parts include but are not limited to, a pulley system with a horizontal belt that powers the crusher. An amputation occurred when an employee reached into the pulley system to attempt to clear a jam. The guard was easily defeated as the employee could easily reach around the backside of the guard. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.
