29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) In Plant #1: Employees were operating a State disc grinder to deburr case cart hinges. The disc grinder was not adequately guarded between the rotating disc and the workbench resulting in nip/pinch points, exposing employees to amputation hazards on or about April 26, 2016. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) At Plant 1: On or about April 26, 2016, an employee suffered an amputation on his right hand (right index finger) as he was operating a disc sander to debur a hinge part. The employer failed to report the amputation within 24 hours to OSHA. CORRECTED DURING INSPECTION.
