29 CFR 1910.101(c): Compressed gas cylinders, portable tanks, and cargo tanks did not have pressure relief devices installed and maintained in accordance with Compressed Gas Association Pamphlets S-1.1-1963 and 1965 addenda and S-1.2-1963:  a) Foaming department, DOT portable tanks, used as pressurized tanks, had pressure relief devices which were not examined by the lead operators to ensure that the device protective covers were not adhered to the device components which may inhibit timely actuation of the devices in the event of over-pressurization, on or about, January 8, 2019.  b) Foaming department, DOT portable tanks, used as pressurized tanks, had pressure relief devices which were not examined by the lead operators to ensure that the exhaust ports of the devices were not coated with paint or other coating which may limit the relief capacity of the devices in the event of over-pressurization, on or about, January 8, 2019.  ABATEMENT CERIFICATION REQUIRED

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:  a) Zinc plating area, metal electrical enclosures, were observed to be heavily corroded such that large scales of rust had formed, thus weakening the structure, on or about, January 8, 2019.   b) Zinc plating area, metal surface-switch retaining rings that were mounted on metal electrical enclosures, were observed to be heavily corroded such that the protective rubber switch covers had fallen off, thus leaving potential vapor openings into the enclosure, and weakening the ability of the enclosure to hold the switch in place, on or about, January 8, 2019.  a) Tin room, a metal electrical enclosure, was observed to be heavily corroded such that large scales of rust had formed, thus weakening the structure, and creating an opening into the box near the wall, on or about, January 8, 2019.   ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area:  a) Foaming Department, the lead operators who periodically entered the tank storage room, had no method to determine if the nitrogen gas used in the process, had inadvertently leaked into the small enclosed room and created an oxygen deficient atmosphere via displacement, on or about, January 8, 2019.  ABATEMENT CERTIFICATION REQUIRED.
