29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Production Area: The ISB Merlin 3000 light curtain system on Cincinnati hydraulic press brake (135-CB-8 #46874) was installed in a way that allowed the operator to reach into the point of operation during the operating cycle, on or about November 7, 2017. b) Production Area: The Link Lite Black Max light curtain system on Cincinnati press brake PB-01 (175AF-10 #49569) was fitted with 6 blanking panels which created a gap in the protection zone that was large enough for the operator to reach into the point of operation during the operating cycle, on or about January 4, 2018. ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED

ABATEMENT OBSERVED DURING INSPECTION CERTIFICATION NOT REQUIRED
