29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)     On or about May 27, 2016, at the laser cutting work cell, the maintenance work that was done on the laser 2 shuttle was done with the shuttle and the overhead gantry crane not locked out.  The lock out tag out procedure that had been developed for the FMS System was not utilized.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:      (a)     On or about May 27, 2016, at the laser cutting work cell, when performing maintenance work on the laser 2 shuttle the employee had not received training regarding the recognition of hazardous energy sources such as unexpected movement of the overhead gantry crane, the magnitude of the energy available from the unexpected movement of the overhead gantry crane and the methods and means necessary for energy isolation and control of the overhead gantry crane.

(b)     At the laser cutting work cell, the laser 1, laser 2, FMS shuttle and overhead gantry crane were not adequately guarded by the chain that hung across the south access point.
