Hwy 51 in McBride, MO 63776 - On or about 06/02/2016, employees were exposed to struck-by or -against hazards where a "Hyster" (Serial No. C114V05548N) powered industrial truck was missing the slip-resistant covering for the brake pedal and had not been taken out of service until restored to safe operating condition.
