First St. Perrysville, OH, On or about June 6, 2016 the employer did not ensure that employee, who was cleaning under the loose install production line,  utilized lockout procedures prior to performing the cleaning. The employee was exposed to caught by and pinch point hazards while conducting cleaning operations  under the loose install line.
