29 CFR 1926.102(a)(2): Eye and face protective equipment being worn by the employees did not meet the requirements specified in American National Standards Institute, Z 87.1-1968, Practice for Occupational and Educational Eye and Face Protection: On or about June 3, 2016, Employees engaged in roofing activities were exposed to a struck by hazard in the eye, while using a pneumatic nail gun on a construction site in Urbana, Illinois, and the employer did not ensure the sunglasses met the requirements of Z 87.1-1968.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee: On or about June 3, 2016, employees engaged in roofing activities were exposed to fall hazards on a construction site in Urbana, Illinois, and the employer did not maintain documentation of the required training for each employee who might be exposed to fall hazards. There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
