Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about August 23, 2017, Bash Roofing Company had employees exposed to falls in excess of 20 ft. without fall protection, while roofing a low slope roof in Champaign, Illinois.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about August 23, 2017, Bash Roofing Company had employees exposed to struck-by hazards on the head without head protection, while working on a low roof section under employees on a higher roof section, at a commercial construction site in Champaign, Illinois. Bash Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.100(a), which was contained in OSHA inspection number: 1187168, citation number 2, item number 1 and was affirmed as a final order on February 15, 2017, with respect to a workplace located at 708 W. Main Street, Urbana, IL 61801. Bash Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.100(a), which was contained in OSHA inspection number: 1064785, citation number 1, item number 1 and was affirmed as a final order on July 1, 2015, with respect to a workplace located at 712 Park Lane Drive, Champaign, IL 61820.
