29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  a.  On or about June 10, 2016, at the administrative offices entrance and exit door area, employees were exposed to a 49 inches fall to the ground below when walking on a stairway that lacked a mid-rail and was used as an entrance and exit access to store main building.  Note:   Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a.  On or about June 10, 2016, at the receiving Class VI area, employees were exposed to an electrical hazard while plugging and unplugging flexible cord with the outer sheeting removed during Crown WP 3035-45 pallet trucks battery charging operation.   b.  On or about June 10, 2016 at the warehouse area next to cardboard compactor, employees were exposed to an electrical hazard while plugging and unplugging flexible cord with the outer sheeting removed during Crown WP 3035-45 pallet trucks battery charging operation. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  On or about June 10, 2016, employees were exposed to a possible fire hazard when working near electrical junction boxes with openings as follows:  a. At the receiving loading dock area, employees plug and/or unplug lamp on junction box with side opening during loading and/or unloading operations.   b. At the mechanical room area, employees work close to junction box with side opening when accessing phone lines panel.    Note:   Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).
