29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a) Jobsite: The employer did not provide employees conducting precast concrete operations with scaffold platforms at least 18 inches wide.  The employees worked from platforms measuring 2" x 6" x 12', approximately 11 feet above ground level, from within the form walls.  Observed on or about 6/10/16.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) Jobsite: The employer did not provide a safe means of access to the platforms of the supported scaffolds inside the form walls by the use of portable ladders, hook-on ladders or other acceptable means.  Employees conducting precast concrete operations climbed the structure of the form walls to access the interior scaffold platforms.  Observed on or about 6/10/16.
