29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures.   Location: 633 Mt, Prospect Ave, Newark, NJ 07104  a.) Employees engaged in residential construction entered and egressed the construction site while having to walk near a breaker box with unprotected live parts 4ft. from the ground. The breaker box and its live parts were not provided a cover for employee protection; there-by exposing employees to electrocution or electric shock hazards.   CSHO observed violation on or about 6/03/2016.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level in accordance with requirements of standard 29 CFR 1926.451(g)(1)(vii): which states each employee shall be protected by the use of personal fall arrest systems or guardrail systems.   Location: 633 Mt, Prospect Ave, Newark, NJ 07104  a.) Employees were engaged in residential construction while utilizing a pump jack scaffold and a ladder jack scaffold without personal fall arrest systems; there-by exposing employees to fall hazards approximately 20 feet to the ground below.    CSHO observed violation on or about 6/03/2016.

Employees were engaged in residential construction, while utilizing a pump jack scaffold and ladder jack scaffold in which the ladder jack scaffold's platform was bridged to the pump jack scaffold; there-by exposing employees to fall hazards approximately 20 feet to the ground below. Employees were engaged in construction activities on a residential roof approximately 25 ft. to the ground below without utilizing any means of fall protection; there-by exposing employees to fall hazards approximately 25 feet to the ground below.     CSHO observed violation on or about 6/03/2016.
