29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Location: 45 Rocco St., Belleville, NJ 07109 - 3rd floor a) Employees were exposed to fall hazards while performing siding and framing activities from a pump jack scaffold, elevated to approximately 20 feet, were the scaffold did not have midrails, end rails, and the employees did not attach lanyards to their fall harnesses. Hazard was observed on or about 3/21/22.

29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems: Location: 45 Rocco St Belleville, NJ 07109 - 2nd floor a) Employees were exposed to fall hazards while performing interior framing operations and accessing different levels using a wooden ramp approximately 15 feet above the next lower level, that did not have a guardrails. The hazard was observed on or about 3/21/22.
