29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location Yasufuku U.S.A., Inc.: The employer failed to ensure that all electrical equipment operated in the area was approved for a CLASS II location. Employees were exposed to a explosion hazard while performing duties involving the manufacture of various plastic parts. A sample taken on July 14, 2016 was determined to be a CLASS II combustible dust. a) Overhead lighting located and operated in a Class II location (Production Area) was not explosion proof and/or approved for the location. T5-6 lamps and IBZ florescent bulbs b) West Point Mechanical fans were utilized in a Class II location (Production Area) and were not explosion proof. Serial # 145334 c) The Shop Vac vacuum utilized for cleaning in a Class II location (Production Area) was not approved for the hazardous location. Model 92L650C: Serial #52747-70

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Yasufuku USA, Inc.: The employer failed to provide training on hazards of combustible dust for employees exposed to a CLASS II combustible material. The employees were exposed to a fire/explosion hazard while performing a variety of assigned duties involving the manufacture of plastic components for motorized vehicles.
