29 CFR 1910.37(b) (4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: a. 2460 Tech Ctr. Parkway, Lawrenceville, GA (Throughout Facility) - Employees were exposed to entrapment hazard. There were several designated, marked, and identified emergency exit doors located throughout the facility which were hidden by large racks of storage materials approximately 25ft. high, which blocks the view of the exit routes, signs and doors. There were no directional exit markings indicating the way to travel, on or about 06/21/16.

29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to the operator that has been observed to operate the vehicle in an unsafe manner: a. 2460 Tech Ctr. Parkway, Lawrenceville, GA (Shop Area) - Employees were exposed to struck-by hazard. An employee was observed performing an unsafe act prior to an incident which occurred on June 6, 2016, such as, loading and transporting a roll of shrink-wrap inside the cab and next to the lift's control, which is forbidden against the company's policy and procedures. The employee was not given refresher training in the relevant topic, on or about 06/21/16.

Employees were not provided with training and information on the types of hazardous chemical/substance that they handle on the worksite such as, but not limited to, Pyroplex (High Performance Grease), Hypuron Motor Oil, Brake Loose (Brakes and Parts Cleaner) and Mystic (Heavy Duty Engine Oil), and how to be familiar with the Safety Data Sheets for the chemicals/substance, on or about 06/21/16.
