29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   a) On or about 6/20/16, in a trench/excavation located on north side of the road in front of 985 East and West Road, West Seneca, NY: Two employees entered and exited a 3 ft. wide x 40 ft. long x 5 ft. 7 in. deep trench (including 8 inches of asphalt pavement at grade) by walking up an inclined ramp of loose stones at the west end of the trench. The north and south banks of the trench were vertical.  Additionally, a 12 inch water main wrapped in a vinyl covering was in the trench and obstructed the path to the exit.  A ladder was laying on the ground near the trench.    NO ABATEMENT CERTIFICATION REQUIRED
