29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about June 28, 2016 employees were not protected from falling through a skylight while working on a roof, exposing employees to a fall of approximately 8 feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
