29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: A powered industrial truck accident occurred on June 9, 2016, that caused an employee to suffer minor injuries. A storage rack was struck by a fork lift and was pushed into the employee. After the accident, the powered industrial truck operator did not receive refresher training.
