29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition or applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: (a) On or about June 29, 2016 at worksite 1435 N Industrial Dr, Houston, Missouri.- Employees were exposed to serious amputation, crushing and struck-by injuries, in that the employer had not identified and trained authorized employees, affected employees were performing servicing activities by changing dies on brake presses which had multiple energy sources (hydraulic, electric, and gravity) including but not limited to Cincinnati Brake Press SN# 52765 known to company as machine 4311-5. without lockout tag out devices. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
