29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On May 6th, 2021 in the brake press area of The Durham Company facility, employees were exposed to amputation or other serious injuries from the brake press, in that the light curtain in place guard did not prevent the operation of the brake press while employees hands are in the operational area of the press.
