29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    At worksite located at 1075 Opportunity Circle, Houston, Missouri.- In the fabrication area, employees were exposed to point of operation, caught in and amputation hazards when working with Amada Brake Press SN# HFB0 80 25 R981213 described by the company as Brake Press 4311-2.   On or about October 3, 2018, a temporary worker received a partial finger amputation injury when operating the Amada Brake Press as it was not guarded.    The Durham Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1159092, citation number 1, item number 4 and was affirmed as a final order on October 17, 2016, with respect to a workplace located at 1075 Opportunity Circle, Houston, MO.
