29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the residential construction site located at 8501 W. 166th Street in Overland Park, Kansas:  Employees engaged in residential roofing activities were exposed to fall hazards of approximately 12 feet 2 inches from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence/structure.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  At the residential construction site located at 8501 W. 166th Street in Overland Park, Kansas:  Employees engaged in residential roofing activities were exposed to fall hazards of up to 12 feet 2 inches from the eave of the residence to the ground below while using a defective ladder to access and exit the roof.  The extension ladder being used at the front of the house had a deformed foot and bent side rail at the bottom on the left side.
