29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located on Lot #164 at 2911 Crystal Springs Point, Gainesville, GA 30507, Roof: On or about 04/15/2025, the employer exposed employees to struck-by hazards while operating circular saws and pneumatic nail guns, in that protective eyewear was not provided while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     a) New residential construction located on Lot #164 at 2911 Crystal Springs Point, Gainesville, GA 30507, Second Floor: On or about 04/15/2025, the employer exposed employees to fall hazards of approximately 13-feet 2-inches to the lower ground below , in that an adequate fall protection system was not provided while engaged in framing activities.

29 CFR 1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:   a) New residential construction located on Lot #164 at 2911 Crystal Springs Point, Gainesville, GA 30507, Second Floor: On or about 04/15/2025, the employer exposed employees to fall hazards of approximately 19 feet 6 inches to the ground level below. An employee tied a knot in a lanyard around a roof truss and connected it directly to the D-ring of the harness, rather than using a proper connector made of forged or formed steel. The improvised method did not meet the criteria for approved fall arrest system connectors.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds per employee attached, or were not designed, installed and used:   a) New residential construction located on Lot #164 at 2911 Crystal Springs Point, Gainesville, GA 30507, Second Floor: On or about April 15, 2025, the employee tied off to a roof truss by knotting a lanyard around it. The truss was not verified or certified as an anchorage point capable of withstanding 5,000 pounds of force, and was not designed, installed, or used in a manner that met the requirements for fall protection anchorage strength. This exposed the employee to a potential fall hazard of approximately 19 feet 6 inches.
