29 CFR 1926.501(c)(3): The employer did not barricade the area to which objects could fall, did not prohibit employees from entering the barricaded area, and did not keep objects that could fall far enough away from the edge of a higher level so they will not fall: On or about July 6, 2016, at the job site located at Ninnescah Wind Farm, Pratt County, Kansas, employees performing work inside the tower were exposed to struck by hazards in that barricades were not established and hatches were not kept closed to prevent objects from falling to lower level work areas.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about July 6, 2016, at the job site located at Ninnescah Wind Farm, Pratt County, Kansas, an employee performing work inside the wind tower was exposed to struck by hazards from falling objects in that a hard hat was not worn.
