29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On July 4, 2016 while cleaning out material from the Chop Saw on line #12, employee was not trained as an authorized lockout employee in that, employee removed excess cut material without turning off/isolating the energy sources and saw blade guard was not in place and/or attached.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Employees who perform cleaning and/or removed of excess cut material on or around the Chop Saw on line 12, were not required to (1) Physically locate and operate the electrical disconnect in such a manner as to isolate the Chop Saw from the energy source. (2) Affix a lockout or tagout devices to the electrical disconnect.
