29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  On or about July 8, 2016, an employee was exposed to the hazard of being struck by flying projectiles, while using  pneumatic staple and nail guns to install roofing shingles without the use of eye protection.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  On or about July 8, 2016, an employee was exposed to a fall hazard greater than 6 feet to the ground level below, while installing drywall and cornices without the use of fall protection equipment or an alternative fall protection plan.
