29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use. a) Throughout the site, 271 Suedberg Road, Pine Grove, PA - A Pettibone B-68 Extendo Rough Terrain Forklift not specifically designed by the manufacturer for elevating employees was used to elevate and transport personnel in an in-house fabricated scaffold platform exposing employees to struck-by and/or fall hazards, as observed on or about January 13, 2016.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied. a) Throughout the site, 271 Suedberg Road, Pine Grove, PA - A Pettibone B-68 Extendo Rough Terrain Forklift traveled horizontally a distance of approximately two hundred-foot (~200') carrying an occupied an in-house fabricated scaffold platform exposing employees to struck-by and/or fall hazards, as observed on or about July 8, 2016.
