29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, were not withdrawn from service until repaired: a) Mortar Mixer- On or about February 5, 2024, employees operating the mortar mixer were exposed to a fall hazard from a fixed ladder with bent rails that was not withdrawn from service. ABATEMENT CERTIFICATION REQUIRED
