29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired. a) 1054 Eisenhower Boulevard Harrisburg, PA - On or about March 14, 2024, employees worked on Hydro Mobile M2 series Mast Climbing Work Platforms (MCWPs) at a height of approximately 22 feet with a missing safety chain point on the Hydro Mobile M2 Bearing Bridge. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) 1054 Eisenhower Boulevard Harrisburg, PA - On or about March 14, 2024, employees who performed work while on Hydro Mobile M2 series Mast Climbing Work Platforms (MCWPs) were not trained to recognize the hazards associated with this type of scaffold, including but not limited to not adjusting the platform height on both motorized unit at the same time when two motorized units are being used in conjunction with a Hydro Mobile M2 Bearing Bridge. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
