29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: When entering the chamber machine at temperatures approximately between 140 to 200 degrees F, in order to free an air line caught on a projection within the machine, employee's arms were exposed to a contact burn hazard if falling onto or contacting any metal surface within the chamber. Personal protective equipment such as protective sleeves or coats were not worn when entering the chamber. Abatement documentation is required for this violation.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and (2) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their area: Employees were not provided chemical hazard communication training when using and exposed to hazardous chemicals, such as but not limited to, solvent based cement, tire paint, and dust generated during buffing and skiving.
