29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) On or about 7/15/2016 - at W044 Bracket Welder machine, employees were required to enter the machine to access the 480V electrical disconnect when utilizing the control of hazardous energy procedure W044 dated June 2016. (b) On or about 7/15/2016 - at W046 Bracket Welder machine, employees were required to enter the machine to access the 480V electrical disconnect when utilizing the control of hazardous energy procedure W046 dated June 2016.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about 7/15/2016 - at W044 machine, guarding was not provided to protect employees from hazards associated with a pinch point when the bracket load door was in the open position to load brackets. (b) On or about 7/15/2016 - at W044 machine, guarding was not provided to protect employees from hazards associated with rotating parts and welding operations due to the light sensors in use were not approved to be used for personnel protection. (c) On or about 7/15/2016 - at W046 machine, guarding was not provided to protect employees from hazards associated with rotating parts and welding operations due to the light sensors in use were not approved to be used for personnel protection.
