29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Southeast, shared materials room, Novatec ABS dryers: on or about July 19, 2016, operators were assigned to regularly transfer ABS resin material from subject equipment into carry totes for plastic injection molding process. The screen guard and perimeter deflector guard were removed from the valve's discharge opening where operators could access hazard area during operating cycle. Operators had their hands and fingers exposed to caught-in hazards likely to result in serious injury.  There is no abatement certification or documentation required for this item.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Southeast, shared materials room, Novatec ABS dryer: on or about July 19, 2016, a machine-specific hazardous energy control (lockout) procedure was not developed and documented for subject equipment when servicing was performed to install two-hand controls and barrier guards.   There is no abatement certification or documentation required for this item.

29 CFR 1910.147(c)(7)(i)(B): Each affected employee shall be instructed in the purpose and use of the energy control procedure.  On or about July 19, 2016, each process technician and machine tender (affected employees) were not trained under the hazardous energy control (lockout/tagout) program in the purpose and use of the energy control procedure when servicing activity was performed.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  Southeast, shared materials room, Novatec ABS dryer: on or about July 19, 2016, mounted electric box for auto and manual function selector was not installed in workmanlike manner where an enclosure was damaged with openings large enough to allow access to live wires. Damaged enclosure was covered by general purpose duct tape.  There is no abatement certification or documentation required for this item.
