29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. On or about November 20, 2025, the employer failed to certify annual periodic inspections of lockout tagout procedures for the Zone #1 Komatsu Press, Cob Dryer, ALMCO Bowl, and Eriez Magnetic Conveyor. Without periodic inspections, deficiencies in lockout tagout procedures and employee practices go unidentified. Employees involved in or around service and maintenance activities are exposed to hazards from unexpected energization, start-up, or release of stored energy which could result in severe injuries, amputation, or death. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about November 20, 2025, the employer did not ensure guards were affixed to the Zone #1 Komatsu Press. There were a set of feeder rollers that pull the metal from the coil as it rotates on a Cooper Weymouth Stock Reel that were left unguarded. The guard was underneath the platform of the rollers. This exposes employees to caught-in/caught between hazards which could result in injuries such as severe lacerations, broken bones, and amputations. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
