29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about December 8, 2025, an employee operating the Printer/Cutter in the Filtration Area was exposed to a rotating fan blade inside the paper shredder while clearing a clog resulting in an amputation; The employer did not ensure that the employee utilized the lockout tagout procedures for clearing clogs on the Printer/Cutter. The HMI Industries Inc. DBA Health-Mor was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 1431603, citation number 1, item number 1 and was affirmed as a final order on Februarys 5, 2020, with respect to a workplace located at 1 American Road, Suite 1250, Brooklyn, Ohio 44144.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 12, 2025, printer/cutter machine operators were exposed to the six rotating parts of the machine to include but not limited to the six rubber treaded wheels while guiding the straps into the PVC piping for the scrapping system. The employer did not ensure that one or more methods of machine guarding were provided to protect the operator and other employees in the machine area from the rotating wheels. The HMI Industries Inc. DBA Health-Mor was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1431603, citation number 1, item number 2a and was affirmed as a final order on February 5, 2020, with respect to a workplace located at 1 American Road, Suite 1250, Brooklyn, Ohio 44144.
