29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided when machines or operations presented potential eye or face injury: (a) Leo Construction, Inc. at 356 Emerson Street, Denver, CO 80218: On or before June 8, 2016, the exposing employer did not ensure employees were provided eye protection when pneumatic nailers were used during roofing operations. The condition exposed employees to the hazard of projectile to the eye.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Leo Construction, Inc. at 356 Emerson Street, Denver, CO 80218: On or before June 8, 2016, the exposing employer did not ensure that employees were protected from injury, in that five employees installing shingles on multi-story residential townhouses, with a roof pitch of approximately 8-in-12, were not utilizing a fall protection system. The employees wore fall harnesses; however, they were not connected to a fall protection system. This condition exposed the employees to a fall of approximately 30 feet.
