29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Leo Construction, Inc., 2900 Purcell Street, Brighton, CO 80602: On and before September 9, 2019 the employer did not ensure that each employee engaged in residential construction activities between 20-25 feet high was protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Employees were conducting roof work and were not tied off. This condition exposed employees to a 20-25 foot fall hazard. Leo Construction, Inc was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1163089, citation number 01, item number 002 and was affirmed as a final order on 8/11/2016, with respect to a workplace located at 356 Emerson Street, Denver, Colorado, 80218.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: a) Leo Construction, Inc., 2900 Purcell Street, Brighton, CO 80602: On and before September 9, 2019 the employer did not ensure that a training program for each employee who might be exposed to fall hazards was created. Employees worked at height at 20-25 feet and were not trained in fall protection. This condition exposed employees to a 20-25 foot fall hazard.
