On or about July 27 2016, employees were exposed to fire and electrical hazards associated with a battery cable connected to the terminal post that was damaged on the powered industrial vehicle and the protective sheathing was missing exposing the bare wires. b. On or about July 27 2016, employees were exposed to struck by hazards because the mast on the powered industrial vehicle was broken and was not removed from service.
