29 CFR 1910.178(n)(8): Under all travel conditions the truck was not operated at a speed that would permit it to be brought to a stop in a safe manner: On or about July 18, 2016, an employee in training to operate a RC-12 forklift was exposed to struck against and caught between hazards in that the forklift being operated in narrow area was not operated at a speed that would allow for a safe stop.

29 CFR 1910.178(l)(3)(ii)(E): Powered industrial truck operators did not receive initial training in work-related topics such as narrow aisles and other restricted places where the vehicle would be operated: On or about July 18, 2016, an employee in training operating a RC-12 forklift in a narrow/restricted area was exposed to struck against and caught between hazards in that the employee was not instructed on the locations where forklifts were not permitted to travel (no drive zones).
