29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it: a) Bridge Construction site, Northern abutment - On or about July 27, 2016, a carpenters' bracket scaffold had deteriorated to a point where the bracket could no longer support four times the intended load, as it failed under an employee's weight, resulting in the employee falling approximately 15 feet to the ground

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a) Bridge Construction site, Northern abutment - On or about July 27, 2016, the competent person had not inspected the carpenters bracket type scaffold from underneath for defects including but not limited to deterioration, wood rot, and delamination of plywood gussets.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a) Bridge Construction site, Northern abutment - On or about July 27, 2016, employees used a fiberglass extension ladder with a split and damaged rail to access a scaffold. The ladder had not been tagged as damaged, nor removed from service.
