29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a). On or about July 29, 2016 one employee was securing pre fabricated roof trusses to the top plate of a 3 story mechanical room and were exposed to falls in excess of 32 feet to the ground below because he was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      b). On or about July 29, 2016 one employee was wearing a fall protection harness however, the life line was secured to an anchor point on the concrete roof deck below the level the employee was working. This configuration would have protected the employee had he fallen away from the structure towards the parking lot but would have caused him to hit the concrete roof deck below had he fallen towards the building.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.  A). On or about July 29, 2016 one employee working at heights above 28 feet was exposed to falls in excess of 12 feet to the concrete roof below because he had affixed his life line and lanyard assembly of his personal fall protection to a "D" ring anchoring device located on the concrete roof structure below the level the employee was working.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    a) On or about July 29, 2016 two employees had climbed the exterior wall framing members to gain access to the roof truss system and were exposed to falls in excess of 9 feet to the roof surface below because a ladder was not provided for employees to access the roof area.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:    a.   On or about July 29, 2016 employees were observed working on the roof truss system of a three story commercial structure. The top rail of the basket of the Articulating boom lift was observed positioned even with the top plate of the exterior wall. Employees climbed out of the basket of the lift and then climbed onto the roof trusses to complete their work.
