29 CFR 1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    The employer is failing to protect employees from the hazards associated with lacerations, broken bones and potential amputations.  During the bending operation of flat corrugated steel into steel irrigation culverts, employees are exposed to unguarded in-running nip points of the bending machine.  The machine was equipped with an activation button on the side of the machine and a partial guard on the end of the rollers.  This allows the machine to be activated with the employees hands in the point of operation.  One instance, which occurred on or about July 26, 2016 involved an employee getting their fingers crushed when they were in the point of operation in the facility located at 4740 E. 39th St., Kearney, Nebraska.
