29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Boiler room office 1D-16A: An orange extension cord was used to provide 120VAC service to a Diversitech 120VAC, 228watt (1.9amp) model CP-22 automatic condensate pump, serial number 14020, mounted on the East wall. The use of a flexible cord in this manner exposes workers in the office to a potential electrical shock hazard. 29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:  Workers were not provided job specific training on the PPE they were using. PPE training includes the selection, use, limitations, storage, maintenance and cleaning of PPE.  (a) In the landscaping shop and locations around the facility: Workers use gas powered Stihl model FSA90R string trimmer, serial number 43551308 string trimmers on lawn areas; Stihl model MS390 chain saws to saw trunks and limbs on or near the ground; and Echo model PPT-265 pole saw pruner, serial number E07811001842 to saw overhead limbs. The workers have eye and face protection for the flying debris created by these tools. The workers also have gloves for hands and chaps to protect legs from the saw blades.  (b) In the HVAC shop and locations around the facility: Workers use oxy-acetylene torches, Milwaukee model 6650 Heavy Duty 120VAC/3.5Amp 18 Gauge Shear, ser. 550A494500085 battery operated hand drills and reciprocating saws. The workers have eye and face protection for flying particles and molten metals, and gloves for hand protection.
