29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: Dermatology Department, Room 3B-68: Employees operated the DAAVLIN 3 Series Light Box to perform patient treatments without personal protective equipment (i.e., UV-rated lab coat). Patients could open the door of the light box while undergoing treatment and employees were exposed to contact with UV radiation hazards.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): Dermatology Department, Room 3B-68: Employees operated the National Biological Hand & Foot II Light Box to perform patient treatments. Employees used a blanket that was not approved by the manufacturer to cover gaps of the light box. Employees were exposed to contact with UV radiation hazards and/or fire hazards due to shield device not-approved by manufacturer.
