29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) Gear boxes for disk screens; On or about June 15, 2016 and at times prior thereto, the employer exposed employees to amputation hazards in that employees were required to do visual inspections on gear boxes containing chains and sprockets where these employees were required to remove the guard to observe the machinery.
