OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by or crushing hazards:   a) On or about May 10, 2017 and times prior at the truck loading area; WestRock Coated Board, LLC as the controlling employer failed to provide a clear and designated path of travel for the powered industrial trucks traveling between truck loading areas #1 and #2 which exposed an employee of Greenbush Logistics, Inc. to struck-by or crushing injuries.  A Taylor TX160 forklift traveling from the railcar loading area to the warehouse to pick up another load started to turn towards the warehouse once he crossed the railroad tracks resulting in striking and running over the right foot of the truck driver positioned approximately 4 - 6 feet behind his trailer assisting his forklift driver in positioning a load onto the rear of his trailer.
