29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)On or about July 20, 2016, on the Peerless Model E-57 Presses and Gralex Press located at Aspen Products, Inc. 3220 Avondale Mill Road, Macon, Georgia: Employees servicing 23 Peerless and one (1) Gralex Press were exposed to amputation hazards while clearing jams without utilizing lockout/tag out procedures to locate and isolate all electric and pneumatic energy of the machine prior to placing any body part into the machine.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about September 15, 2016, at Aspen Products, Inc., 3220 Avondale Mill Road, Macon, Georgia, production floor: a) Employees performed servicing of Peerless Model E-57 Presses to include, but not limited to clearing jams without utilizing Lockout/tag out procedures were exposed to amputation hazards and one employee was injured amputating his right middle finger while clearing a jam from the Peerless Model E-57, Serial Number E5-1004-15, when lockout/tag out procedures were not utilized. b) An employee performed servicing of a Peerless Model E-57, Serial Number E5-1012-15 without utilizing Lockout/tag out procedures was exposed to amputation hazards when he stood inside the press to clear a jam and thread the paper into the press.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about July 20, 2016, on the Peerless Model E-57 Presses and the Gralex Press located at Aspen Products, Inc., 3220 Avondale Mill Rd, Macon, Georgia: Machine operators performing servicing to include, but not limited to unjamming of the 23 Peerless Presses and 1 Gralex Press were exposed to amputation hazards when the operators were not trained as authorized employees in the recognition of hazardous energy sources and the methods and means necessary for energy isolation while performing authorized level service work on the presses. Aspen Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(I)(A),which was contained in OSHA inspection number 1050453, citation number 1, item number 2, and was affirmed as a final order on July 7, 2015, with respect to a workplace located at 3220 Avondale Mill Rd, Macon, Georgia.
